In partnership with The Connecticut Department of Public Health and Connecticut State Department of Education, Wooster School has arranged to have an on campus COVID-19 Vaccine Booster Clinic on Thursday, January 13, 2022 from 4:00 - 7:00 pm in the Dining Room. Pfizer, Moderna, and Johnson & Johnson vaccines will be available.
Due to the availability of spaces, we have now opened this clinic to all eligible community members (faculty, staff, parents, siblings, and students ages 12 and older). Those who are interested in attending, please complete this form by Thursday January 13, 2022 at 8:00 am. Signing up will reserve your spot, but the clinic itself will not have reserved appointment times. This form will close when all slots are taken.
A reminder that boosters are available to persons that are 12 years of age and older who:
- Received Johnson & Johnson COVID-19 vaccine 2 or more months ago
- Completed the Pfizer-BioNTech COVID-19 vaccine primary series 5 months or more ago
- Completed the Moderna COVID-19 vaccine primary series 5 months or more ago
Heterologous doses, mix and match dosing, for a single COVID-19 vaccine booster for eligible persons is approved. Please speak with your provider to decide which is best for you. COVID-19 Vaccine Booster Shots | CDC
Please bring documentation of initial COVID-19 vaccine doses. Insurance cards and identification are requested, but vaccines will be given at no cost to the individual regardless of insurance status. Please complete the Patient Acknowledgement and Attestation Form and pre vaccination checklist prior to attending to expedite the vaccination process. The parent/guardian of the child does NOT need to be present, but the paperwork needs to have a consenting signature from the parent/guardian and be fully completed. Completed paperwork must be presented at the clinic.
Anyone under isolation or ill at the time of the clinic, will not be eligible to attend. Anyone who has been diagnosed with COVID-19 illness must be fully recovered, symptom free, prior to receiving a booster dose.
Once your student has received the COVID-19 vaccine, please upload the documentation into Magnus Health.
